Shift/Identity New Student Form

Shift and Identity are our Middle School and High School Programs at Life Stream Church. We meet from 6:30-8PM on Sunday nights. Shift: Grades 6-8, meet in the Hub through front Entrance B, and Identity: Grades 9-12, meet in the Worship Center through Entrances C and E.
